You're Care is Our Biggest Priority

Medicaid long-term in home care programs for seniors, adults, and children with disabilities in Wisconsin. We accept Medicaid/Forward Health, Community Care, Community Care Family Care, iCare, Molina, Premier, GT, VA, and IRIS programs at no cost to the client for our services. Private Pay is available if one of the listed programs is not an option. BLS Home Care offers Self-Directed Personal Care Services and Agency Based Personal Care Services, along with Supportive Care options as well.

Who Is Eligible?

We can help you determine if you may be eligible for program provided Personal Care Services.
Qualifications include:

Live in Southeast Wisconsin
Be at least 5 years old
Be a senior, adult, or child with a disability
Not living in a long-term care facility or nursing home.
Have a primary care doctor that you have visited in the past year.

What Services Are Available?

Depending on eligility and program approval, self-directed personal care services and agency based
personal care services may include assistance with:

Bathing and/or personal hygiene
Dressing and grooming
Eating and meal-related assistance
Toileting
Mobility and transfers
Medication reminders
Other approved personal care needs

How It Works

Self Directed Personal Care Services

With BLS Home Care’s Self Directed Personal Care Services you will be able to choose, supervise, train and manage your personal caregiver. Your caregiver may be a family member, friend, neighbor, or anyone of your choosing. And you will be able to coordinate your care around your:

Agency Based Personal Care Services

With BLS Home Care’s Agency Based Personal Care Service we will provide you with a qualified caregiver that has been through our rigorous background check and training with a BLS RN Nurse to provide quality care in the comfort of your own home. We build your schedule and care plan around your needs, and our nurse coordinates with you and your doctor to ensure your needs and wishes are being met.

Private Pay Personal Care Services

With BLS Home Care’s Private Pay Personal Care Services we will provide you with a qualified caregiver that has been through our rigorous background check and training with a BLS RN Nurse to provide quality care in the comfort of your own home. We build your schedule and your plan around your needs and wishes. We offer flexible scheduling with as few as one day per week, or as many as seven days. It’s your choice.
