BLS HOME CARE Self Directed Personal Care Service can also be used in other settings outside of the home, such as doctor offices, shopping and errands, and assistance to activities. 

You will be able to choose, supervise and manage your personal caregiver. You can choose a family member, friend, or neighbor – any person of your choosing, provided that they meet the criteria. If you want more control over your Personal Care Services, this might be a good option for you. 

The BLS Home Care Self Directed Personal Care Services is designed in a way that gives you freedom of choice while providing assistance to be sure that you stay safe and healthy in the comfort of your own home. A BLS nurse works with you to set up a client specific plan of care. Your BLS nurse will provide ongoing in home oversight and do all of this in coordination with you and your doctor to ensure that your personal care plan and ongoing care meets all of your needs and wishes.

If you have a Medicaid/Forward Health Card, you may be eligible for Personal Care Services. If you already have services through a Medicaid Personal Care Agency, you already qualify. In addition, if you have iCare, Molina, Family Care, or Community Care, you may also qualify.
